orders over $100. You must first fill out the
online form to get the total amount,
mailing address, and fax number. Be sure to select "Purchase Order"
as "Method of payment" on the order form. This option will not be
available until the total price is at least $100, which means you
have to order multiple copies.
The second page of the online form contains instructions on how to
complete your purchase. Be sure to print the page and include it
when you mail or fax your purchase order.

15 minutes of completing your order. If you don't receive it
within this time, check any spam filters or holding areas for your email account or

to your email address book or list of allowed addresses to make sure you receive it.
If you still haven't received your registration message within a reasonable
time or have determined that a spam filter deleted it, please
send a support request. Please note that our response to your request may also be filtered if you have not fixed the problem!
